Changes to our Primary Care Network (PCN) from 01 April 2025

Dear Patients

With effect from 01 April 2025, Denton Village Surgery will no longer be associated with ParkWooD PCN. Denton Village Surgery and Wootton Medical Centre will be joining MWEB PCN.

MWEB PCN is a Primary Care Network currently comprising of Moulton Surgery, Woodview Medical Centre, and Kingsthorpe Medical Centre. In addition, Denton Village Surgery and Wootton Medical Centre will be joining the team!

In total MWEB provide services for over 30,000 patients.

Since the NHS was created in 1948, the population has grown, and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.

To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

PCNs are led by clinical directors who may be a GP, general practice nurse, clinical pharmacist or other clinical profession working in general practice.

Please rest assured patient care will not be compromised by this move. We will be working together with MWEB PCN to improve patient care and services we offer.

MWEB PCN are a progressive thinking and supporting team who strive to provide safe, cost effective, evidence-based care to all our patients.
